Job Title: Band 6 Community Psychiatric Nurse (CPN) Trust: Essex Partnership University NHS Foundation Trust Location: Recovery & Wellbeing – Taylor Centre, Warrior House, 42–84 Southchurch Road, Road, Southend-on-Sea, Essex, SS1 2LZ Days: Monday to Friday Hours: 09:00 – 17:00 Duration: 4 Weeks (Possible Extension) Start Date: As soon as possible
Description: We are currently recruiting experienced Band 6 Community Psychiatric Nurses (CPNs) to join the Recovery & Wellbeing Team at the Taylor Centre in Southend. This is an excellent opportunity for skilled mental health professionals who are experienced in community mental health and care coordination.
Key Responsibilities: • Manage a caseload of up to 35 patients with severe and enduring mental health conditions • Act as Care Coordinator in accordance with the Care Programme Approach (CPA) • Complete comprehensive mental health assessments and risk assessments • Develop, implement, and review person-centred care plans • Work collaboratively with the multidisciplinary team and external agencies • Provide therapeutic interventions and ongoing support to service users • Maintain accurate clinical records and documentation in line with NHS policies • Promote recovery, independence, and positive mental health outcomes
Requirements: • Valid NMC Registration (Registered Mental Health Nurse) • Previous Community Psychiatric Nursing (CPN) experience • Experience managing a community mental health caseload • Knowledge of CPA, risk assessment, and safeguarding procedures • Up-to-date mandatory training • Enhanced DBS • Available to start ASAP
